By submitting this form, you are giving permission for your child to participate at the Level Up Sports Event You hearby release and hold harmless any coaches, staff, Trainers or sponsors and all associated individuals or members from loss, injury, liability or expense. I grant Level Up, its representatives and employees the right to take photographs of me and my property in connection with the above- identified subject. I authorize Level Up, its assigns and transferees to copyright, use and publish the same in print and/or electronically. I agree that Level Up may use such photographs of me with or without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising and web content. I understand Level Up will not sell and or give away photographs of me to any third party companies, without my written consent.
